Safely Prescribing GLP-1 Medications for Older Adults

Safely Prescribing GLP-1 Medications for Older Adults: Remote care solutions that offer protection and connection 4 min readTransforming Obesity Treatment with GLP-1s for Older Adults A recent survey conducted by Gallup reported that perceived effectiveness of weight loss injections are relatively consistent among younger and middle-aged adults, with 70% of those aged 18 to 49 and 67% of those aged 50 to 64 reporting them as effective or extremely effective. Among adults aged 65 and older, however, reported effectiveness is much lower, at 48%. 48%。 Older adults often face more health issues or comorbidities, which can make it challenging for them to lose weight and adhere consistently to their treatment plans. Additionally, healthcare providers may encounter greater difficulties in keeping older adults engaged and motivated throughout their weight loss journey. This age-related disparity points to the need for more tailored treatment plans, closer monitoring of the patient’s day to day health metrics, and additional research to better understand the overall effectiveness of GLP-1 medication to treat obesity in older adults 這種與年齡相關的差異表明需要更量身定制的治療計劃、更密切地監測患者的日常健康指標以及進行額外的研究以更好地了解 GLP-1 藥物治療老年人肥胖的整體有效性Mitigating medication risk with remote health tech 透過遠距醫療技術降低用藥風險One approach to addressing these challenges is the integration of digital care solutions, which can offer greater protection for older patients. By utilizing remote monitoring technologies, healthcare providers can track vital health metrics in real-time, allowing for prompt adjustments to treatment plans as needed. These tools can also facilitate regular communication between patients and providers, ensuring that any concerns or side effects are quickly addressed, thus enhancing the overall effectiveness and safety of the treatment. With connected health devices that collect patient-generated data on a daily basis, care teams can have more insight into health risks that older patients are more susceptible to when taking GLP-1s: Sarcopenia: About a quarter of GLP-1 agonist weight loss results from the loss of muscle and bone, both of which are necessary to protect us as we get older. Although some muscle loss is a natural part of aging, it accelerates while on GLP-1s. Weakened muscles can make everyday activities difficult, and can lead to falls or other injuries. Low muscle mass is also associated with poor health outcomes — researchers found that individuals with diabetes and low muscle mass have a higher risk of mortality from cardiovascular disease. Malnutrition: Although suppression of appetite is a key mechanism of GLP-1 medications, too much appetite control can be dangerous, particularly in older adults who are more prone to having reduced food intake. Studies have found diminished appetite is a common problem for older adults — both those living at home and at care facilities. Early insight into a patient’s weight and body composition can help care teams intervene earlier to promote better appetite and food intake. Low muscle mass further compounds the risk of malnutrition. A study led by the Center for Translational Research in Aging & Longevity at Texas A&M University found that current care approaches for malnutrition often don’t account for the importance of muscle mass restoration. Researchers recognized the challenge of measuring body composition in clinical practice, noting “the need to raise awareness of the importance of screening and managing ‘at risk’ patients so it becomes routine is imperative for change to occur.” Gastrointestinal side effects: Severe gastrointestinal side effects from these medications can result in dehydration, which may increase the likelihood of falls. The most commonly reported side effects include gastrointestinal issues such as vomiting and nausea. When dehydration occurs, individuals are more prone to dizziness or fainting, raising the risk of falling. For older adults a fall could be the start of serious health issues and can even lead to a long-term disability. It is essential to ensure that older adults taking weight loss medications can monitor their hydration levels on a daily basis. Daily monitoring of lean mass, water percentage and bones mass With the Withings Body Pro 2 cellular scale, patients can weigh in each day at home and their body composition information will transmit to their care team automatically. This scale was designed for maximum ease of use, making it ideal for older patients who may not be tech savvy — there’s no technical setup required, and patients can use it even if they don’t have a cell phone or internet service. Daily lean mass, fat mass, water percentage and bone mass metrics provide early insight into which patients are struggling with muscle loss or malnutrition. With this data teams can more quickly identify problems and reach out to those in need. For instance, encouraging patients to increase protein in their diet, drink more water, and do light strength training may help stave off serious health issues. According to the Pew Research Center, about 27 percent of American seniors live alone. This puts them at higher risk of social isolation, which can affect their well-being and increase their risk of health problems. Beyond measuring body composition, the Body Pro 2 scale can deliver motivational messages to patients automatically. These messages might offer encouragement to keep walking regularly, take their readings more consistently, remind them of upcoming appointments or congratulate them for reaching a personal health goal. Prioritizing a personalized approach Safely prescribing GLP-1 medications for older adults requires a multifaceted approach that combines the integration of connected health devices, closer monitoring of patient body composition, and consistent patient motivation. These technologies not only enhance the safety and effectiveness of weight loss treatments but also provide motivational support to help older patients stay engaged and connected. By addressing the unique challenges faced by older adults, healthcare providers improve the success of their program. Disclaimer: The information provided in this article is informational and for content marketing purposes only. Understanding our ESC and DPN

Understanding our ESC Technology for Detecting and Monitoring DPN 4 min readElectrochemical Skin Conductance (ESC) is a physiological parameter that measures the reactivity of sweat glands and small nerve fibers in the skin to electrical stimuli. Elevated blood sugar levels can harm blood vessels, inhibiting their ability to supply oxygen and essential nutrients to these small nerves, leading to their deterioration. This effect is amplified because the nerve fibers that supply sweat glands are long, thin and unmyelinated, they are easily damaged. When the sweat glands and small nerve fibers lose functionality, it is a sign of starting peripheral neuropathy. Diabetic peripheral neuropathy (DPN) affects the majority of patients with diabetes, but it is difficult to diagnose in its early stages because up to half of those affected are asymptomatic (no pain and good feet sensation). These undetected signs and lack of care can lead to Diabetic Foot Ulcer (DFU) and in the worst case to amputation(1). Though late stage DPN cannot be reversed, early detection can help to slow the progression through exercise, HbA1c control, addressing certain vitamin deficiencies, and other lifestyle changes. According to the American Academy of Family Physicians, early detection and treatment of DPN and resulting foot ulcers has the potential to prevent up to 85% of amputations (2,3). (2,3)。   Many studies have demonstrated the link between sudomotor function and the risk for DPN paving the way for the use of the Sudoscan within DPN characterization (4,5,6,7). ESC has now a 15+ year track record of R&D and a body of peer-reviewed research in more than 200 scientific publications. Because of this vast array of clinical evidence,Withings decided to integrate the ESC measurement into its scales leading to the Body Pro 2 device. To measure ESC, patients simply step on the Body Pro 2 scale (8). Electrodes in the scale emit a small painless current to the feet, and the device measures the resulting chloride conductance in the sweat glands. Within 60 seconds, patients and their providers receive their ESC score (and other biomarkers). Low conductance, and thus a low ESC score, indicates sudomotor dysfunction with a known threshold to stratify patients. With a persistent DFU and amputation problem impacting millions of people with diabetes, it is clear that current clinical methods and patient compliance are insufficient to markedly reduce incidences. Even for patients who have their annual foot exam, the conventional monofilament test results in a misdiagnosis nearly half the time (9). Skin biopsies are conclusive, but invasive, painful, and especially problematic for patients who have a high risk of infection and whose wounds heal slowly. By contrast, our ESC technology provides a rapid, operator-independent, and reproducible method that can replace monofilament and balance the drawbacks of invasive biopsy. While preventing DFUs is a complex problem requiring multi-faceted solutions, we are already working with leading providers in the U.S., Europe and Asia to enable easier and more reliable diagnosis and monitoring of DPN using the ESC technology in Body Pro 2. Importantly, we believe that the quantifiable, reproducible, rapid and non-invasive methodology has far more potential for scaling in proportion to the size of the DFU problem, and can better serve the requirements of a highly diverse patient population. References Armstrong, D. Shifting gears: How Giles Navigates Heart Health

Giles Hudson’s life took an unforeseen detour when he fell ill in early 2020, leading to a battle with lingering symptoms and a serious heart condition. Thanks to his perseverance he emerged as “Giles 2.0”. Read more about his experience and resilience. Giles Hudson, 55, lives about five miles north of Coventry, in the heart of England. A trained marine zoologist who shifted gears to become a marketer, Giles fuels his passion for cars as a test driver for the Motor Industry Research Association (MIRA). He’s an expert in control behind the wheel of Jaguars and Land Rovers, navigating the twists and turns of the road with precision. But much like a test track, life can throw unexpected detours our way. One day, Giles’ journey took a sharp and unforeseen turn, reminding us that no matter how much we steer our lives with confidence, we’re not immune to unpredictability. In late February 2020, on the cusp of the pandemic, Giles fell ill. He battled a high fever for over ten days and, although he never required hospitalization, he says the illness left him with lingering symptoms. “It was a complete turnaround,” Giles recalls. “I’ve always been reasonably healthy. I don’t drink, don’t carry a lot of weight, and I’ve never had any issues with my heart.” But the aftermath of the virus was a different story.” Giles found himself plagued by exhaustion, brain fog, and an inability to concentrate. As the months passed, Giles’ physical health did not improve. He began to experience anxiety and depression, and it was unlike anything he had ever been through. “I was the one at the end of the soap operas on the telly where they say, ‘If you’ve been affected by anything you see on this program, call the 800 number…’ That’s what I felt.” His doctor, recognizing the severity of his condition, signed him off work for three months, prescribing rest and time outdoors. But recovery took longer than expected, and it wasn’t until August 2021 that Giles felt ready to return to work at the University of Coventry’s marketing department. However, a startling observation one morning—in which he saw a visible heartbeat pulsing through his shirt—prompted an urgent visit to the doctor. After an ECG, Giles was admitted to the hospital with a heart rate of 155 beats per minute. He was given a diagnosis of atrial fibrillation and mitral regurgitation, meaning his heart was not efficiently pumping blood, leading to pooling and an increased risk of clotting. During his month-long hospital stay, Giles faced a critical decision regarding his treatment. “They offered me a mechanical valve, a pig heart valve, or to repair my own,” he explains. Due to his young age, the doctors advised that the repair would be the best bet for him, as it would last the longest out of the three options, so he went with that. It was an invasive procedure and required a lengthy recovery at his parents’ home followed by a rigorous rehabilitation program. Giles said he left the hospital as “Giles 2.0” and in May 2022, he decided to invest in his health by getting himself a ScanWatch Horizon to monitor his heart, saying “It was like treating myself to a new version of me.” In December 2023 that investment truly paid off. In a few instances where Giles’ heart rate remained elevated despite medication, he received high heart rate notifications on his smartwatch, which prompted him to seek medical attention. During his appointment, he showed data from the Withings app to his care team, which helped them make informed treatment decisions, including increasing his dosage and scheduling a procedure for his atrial flutter. As is par for the course when you experience a life-changing event, Giles had a huge shift in perspective, “Money is important, but it’s not the end all be all. It’s only after something like that, that you realize health is everything.” Reflecting on his experience, Giles offers advice to others who may be navigating similar health challenges. He says you might benefit from it “for the reassurance” and adds, “if you have had cardiac issues particularly, think of a ScanWatch as a silent friend constantly looking out for you.” Giles’ journey is a reminder of the resilience of the human spirit. When life takes its unexpected—and inevitable—twists and turns, we can adapt and overcome. And it doesn’t hurt to have a silent guardian on your wrist watching over your heart, every beat of the way.

Lessons in health technology

Erik’s journey into connected health began with a smart scale acquired through an insurance incentive, which led to the creation of a comprehensive ecosystem of health devices. Read more about how Erik seamlessly integrated smart scales, watches, and thermometers into his daily routine, enhancing health tracking for himself and his family. We met Erik Van Houten, 33—a devoted educator, athletic director, and family man living in Las Vegas—when he stopped by our booth at the 2024 Consumer Electronics Show (CES) in the Las Vegas Convention Center. A long-time fan of Withings connected devices, he weaved health tracking technology into the fabric of his daily life, creating a seamless and positive impact on his health. Originally from Rochester, NY, upon graduating college, Erik Van Houten was offered a position in Las Vegas through Teach for America—a non-profit organization that recruits and trains highly qualified college graduates and professionals to teach in low-income communities to help combat educational inequity. Erik arrived in 2012 and he’s been there ever since, teaching 12th graders in East Las Vegas AP Government and university students Intro to Political Science at UNLV. Juggling these roles with family life, Erik discovered a connected health ecosystem to help maintain his well-being and support his active lifestyle. How it started 它是如何開始的Erik’s first encounter with a smart scale was via a Withings one he got in May 2019, a choice motivated by a health insurance incentive. The convenience of syncing data to the app made things easy. Wanting style to match the substance Along with his scale, Erik had a basic fitness tracking band. But after living with it for five years Erik realized he wanted a smartwatch that complemented his style without being intrusive. One day he Googled “Smartwatches that don’t look silly” and he saw that the same company that made his scale also offered a stylish smartwatch. “I was after something cool,” Erik reminisces, “a watch that didn’t bombard me with alerts.” The promise of extended battery life meant he could track his sleep without the nightly charge ritual. When the ScanWatch became available, Erik was quick to upgrade. Its classic design and health-tracking features won him over, making it his go-to accessory for any occasion. “It’s amusing at weddings when you see someone in a suit wearing a less discreet smartwatch,” he laughs. Erik has always been active and athletic, but his ScanWatch has prompted him to be even more competitive, pushing him to surpass his previous day’s achievements. “Seeing the data and trends over time in the app… It’s revolutionized how I view my health,” he shares. Erik noted how accurate the step counter on the ScanWatch is, and how it’s a stark contrast to his old fitness tracker. “With Withings, reaching 10,000 steps is a genuine accomplishment,” he asserts, noting that his previous tracker would count arm movements as steps, resulting in exaggerated numbers. This accuracy would someday serve another purpose. When Erik got an unexpected blood pressure result at the doctor’s office, he knew just where to go to help get to the bottom of it. Blood pressure peaks and nervous streaks The one device that gets used the least, but could be said to have a pretty high payoff is Erik’s smart blood pressure monitor, the BPM Connect. During his annual physical, Erik would get a high blood pressure reading every year, causing a bit of concern with his doctor. However, he had a gut feeling that it wasn’t an issue and sought proof that he was OK. Being familiar with the Withings ecosystem, he felt he knew where to find an easy-to-use blood pressure monitor that would sync all his data to the same app where all his other data was living. Erik explains, “I remember the directions were one of the best times you should do it is right when you wake up in the morning. I did it right in the morning a few times and it was perfectly fine. I was like, OK, I’m good.” Confirming that it was what he expected—a case of white coat syndrome where he was just nervous at the doctor’s office—he showed his doctor his blood pressure results in the Withings app. It’s since streamlined conversations with his doctor, “Now I can get straight to the point with my doctor, backed by reliable data,” he explains. Little did he know that wasn’t the only device that would help him in an unusual circumstance. How a tool from his medicine cabinet became a pandemic game-changer As athletic director at the high school, Erik faced the challenge of ensuring the safety of his soccer team during the pandemic. With strict procedures in place nationwide, no-contact thermometers were in high demand. Thankfully, Erik had also purchased a Withings Thermo through his health insurance incentive program before the pandemic, and it emerged as an unlikely hero—allowing Erik to quickly and accurately check each player’s temperature before games. It became a staple in his toolkit, providing peace of mind and allowing the team to focus on the game, not the virus. Devices that adapt to a growing family A lot has changed since those hard times during the pandemic, in more ways than one. Erik and his wife now have a 16-month-old daughter. His wife got on the Withings bandwagon during her pregnancy using Pregnancy Mode on their smart scale and then graduated to Baby Mode once their daughter was born. And now, the whole family can use it. The next step in home health monitoring Always finding new ways to stay active, Erik is taking skating lessons to better his hockey skills and is excited to put his ScanWatch to use during the upcoming track and field season with his high school students. We asked him what he would want next from Withings and he referenced the new home health/telehealth device he saw demonstrated at the Withings CES booth. “I think what I’m excited about is the BeamO…with BeamO you can say, ‘Here’s all the information’, and it’s just so much quicker than having to see the doctor in person. I am definitely getting BeamO when it comes out. I have my eye on that.” Learning how Erik has seamlessly woven Withings into his life—from personal health to his work and family—was seeing our vision come to life. His dedication to education and well-being inspires us to stay true to our mission of creating simple-to-use, smart solutions that support meaningful lives.

A Banker Invests In His Health:

A banker from Rheinhessen, a well-known wine region in central Germany, wakes up one morning in November and decides to lose weight. And he actually follows through -— 53 kg (117 lbs) in just 10 months. No joke. How did he do it? Get to know his ambitious plan, the beginnings, the difficulties, the aids and the successes. On November 25, 2022, Kay von Dreger, 54, woke up and officially declared war on his excess weight. He decided to lose weight because the physical strain was getting to be too much. At that time, he weighed 151 kg (332 lbs), and his BMI was over 40—extremely obese. He also had high blood pressure, which was already being treated with medication. Kay had always been a big guy, but the amount of excess weight had accumulated gradually over the years, mainly due to snacking, stress eating, lack of exercise, and long working hours. Kay knew it wouldn’t be easy. He didn’t believe in miracle diets; he was familiar with the yo-yo effect. He even briefly considered surgery until his wife encouraged him to give it a try on his own. But just giving it a try is not Kay’s style — he is an analyst and realist through and through. He describes himself as a nerd who is very interested in data and also challenges it. So Kay went right into the thick of things on November 25. First, he created a severe calorie deficit by restricting his daily calorie intake. In practice, this meant no carbohydrates, lots of vegetables, lean protein, i.e. only poultry, and an absolute ban on snacking—and all this so close to Christmas! When asked how he managed to get through the holidays with this low-calorie meal plan, he said that a lot of it is a matter of the mind: “You have to really want it. Sure, I could have been tempted by the smell of freshly-baked Stollen. It was already like a kind of gluttony that you first have to get away from. Comparable to smoking.” With a clear goal in mind, Kay proved to have an iron will. By the end of December, he had lost the first 15 kilos (33 lbs) — mostly water, as he could tell from his body composition readings on his Withings smart scale. This was also how he explained the rapidity of the weight loss. It was a fantastic first success that also caught the attention of those around him. In particular, acquaintances he hadn’t seen for a while were the first to notice that his face looked thinner. To make further progress, Kay introduced more exercise into his daily routine from the end of December. Here, too, he didn’t do it halfway: He set himself a daily goal of 10,000 steps right away. He tried to walk these mainly during the course of the day. If he didn’t manage to make his rounds during his lunch break, he’d do it in the evening. This worked well at first, but the motivation decreased over time, because that many steps took about 1.5 hours to complete. Logging that time in the evening was tough, and he missed the time with his family. So he looked for a more efficient solution. He was able to live out his motto “Measure, weigh, assess” in detail with the Withings app—the visual representation of his development and the detailed measurement data showed him exactly where he was. This made it easier for him to plan further steps and to target his efforts effectively. Thanks to daily use of his smart scale, he noticed that in addition to losing water and fat mass, he was losing muscle mass as well. So, in the summer of 2023, he put his step goal on pause and went to the gym twice a week for 20-minute treadmill sprints and lots of strength training. Kay’s gym had machines that adjusted to his weight and goals, which helped him be his own trainer. He then relaxed his calorie restriction a bit which slowed his weight loss down. It was also clear to him that he couldn’t follow an extremely strict regimen forever. It was a successful starting shot that got the kilos rolling, and now it was a matter of readjusting so that his goal remained realistic and sustainable. An important milestone for Kay was to get below 100 kg (220lbs), which he reached in June 2023. According to his own statement, this was “a special event that touched my soul.” To his great delight, he went down 8 shirt sizes, and he no longer had to buy his clothes in specialty shops that sold only big and tall sizes. The other day, he was sorting out some clothes and had a jacket in hand that he and his wife now easily fit into together. “Those are the kind of moments that make me proud of myself and motivate me to keep going.” Looking back, Kay credits two factors for his weight loss: completely cutting out carbohydrates (except for sushi on occasion) and measuring his weight daily with his Withings smart scale, which also analyzed body composition. Kay told us he didn’t buy a scale when he started this journey… in fact he owned a Withings smart scale for many years, and it was used by the whole family. But for the year that showed him great progress, his scale did play a starring role in Kay’s everyday life. When asked how he feels and what has changed in his life, Kay reflects: 當被問及他的感受以及他的生活發生了什麼變化時,凱反思道: “When I weighed 150 kg, I couldn’t walk 9 km. Now I hop like a young deer, I’m much more agile and my blood values are in the positive range. Soon I’ll have an appointment with a cardiologist, and I’m really looking forward to it.” What’s next for Kay? Currently, the scale shows 98 kg and he says he’s on a weight plateau, which annoys him a bit, he’d like to reach his goal a bit faster. He has lost a lot of weight on his face and all over his body, but the fat is holding on in the abdominal area. “I want to get to 80 kg and only when I can keep the weight off for two years, would I call it a success.” At the same time, Kay also knows that once he reaches his goal, he can’t just stop and go back to old habits, because then the dreaded yo-yo effect can happen. It’s a change for good. However, Kay is positive about this. He is approaching 55 and wants to stay healthy and fit for his family for a very long time. To do this, he is happy to forgo overindulgence in empty calories. Which doesn’t mean he doesn’t indulge at all: “I love pistachio ice cream, and I haven’t given it up in the summer.” And that’s a good thing: In life, realistic goals, ambition and effort are just as much a part of life as celebrating and enjoying it. It’s all in the balance! We thank Kay for his story and wish him continued success on his health journey.
